We have a number of services that individuals can access to address
their serious illness as part of their overall continuum of care:
Home Delivered Meals
for the Seriously Ill:
For those who have a
serious illness and are physically limited in their activities of
daily living. We also provide meals to any dependent children under
the age of 18 that are under your care.
Nutritional Counseling and Education:
By registered dieticians
that provide nutritional counseling and education by phone to support
individual medical needs and treatment regimens (in consultation
with primary medical providers), including a complete nutritional
assessment and analysis of food tolerance, allergies, treatment
side affects, etc.
This service does not
require enrollment in the home delivered meal program. Please call
Client Services to enroll at 1-800-747-2023
To refer yourself
to God's Love We Deliver, please follow these instructions:
- Call our Client
Services Department at 1-212-294-8102 or 1-800-747-2023 and identify
yourself as a potential client. A staff person they will determine
your eligibility and schedule a time to do a telephone intake
interview. If
you reach our voicemail, press "1 " and leave a message
with the spelling of your name, your telephone number, your zip
code and a good time to reach you
- Prepare for
the intake interview by having a list of any symptoms or allergies,
your medications and your medical provider's name, telephone number
and fax number with you.
- After the intake interview
is completed you will be scheduled for your first meal
delivery
- Sign your paperwork
on the first day you receive a meal and return it to the delivery
person
- We will fax an eligibility
form to your medical provider for them to complete, sign
and fax back to us within 10 business days
To refer someone else to GLWD take the following steps:
- Have the person with
you when you call or call after you have faxed a signed Release
of Confidentiality Form to 1-212-294-8198 (see Forms link to select
the appropriate form)
- Follow steps 1 through
6 at the top of this page.
