Home Delivered Meals for the Seriously Ill:

To be eligible for our Home Delivered Meal, a person must meet the flowing criteria:

• Live in New York City or Hudson County, New Jersey;
• Have a medical diagnosis of a serious illness such as Cancer, Alzheimer's disease, Parkinson's disease, Multiple Sclerosis, HIV Disease/AIDS, ALS or other serious illness
• Physical difficulties due to your illness that restrict your activities of daily living. (Such as difficulty walking, standing for long periods of time, etc…)

All clients must obtain a valid eligibility letter within 10 business days of completing a phone interview with our staff to continue to receive meals. The letter verifies the client's medical diagnosis and that they are restricted in the activities of daily living.

Our eligibility letter must be completed and signed by a doctor, a physician's assistant, or a nurse practitioner. Please refer to the Forms link for a pdf of the form.

Women's Nutritional Support Program

Eligibility Criteria:

• Women diagnosed with Ovarian, Cervical or Uterine Cancer
• Who are beginning or are already receiving chemotherapy and/or radiation treatment
• Living in East, Central or West Harlem (Within zip codes: 10026, 10027, 10029, and 10035)
